The Washington State Medical Marijuana Authorization Agency LLC is a compassionate Seattle based company which provides medical marijuana authorizations to patients who qualify. 

 First things first, if you feel your condition meets the requirements, all you need to do is click on the Forms link above. You will be directed to our forms page where you can fill out the Patient Intake Form and Informed Consent Form. They are needed in order to see the doctor. The authorization visit is 10-15 minutes, and you will have your authorization when you walk out the door. 

Our authorization base price is $200 for first time medical cannabis patients, $175 for renewals, and $200 for six month authorizations. Six month authorizations are for people with little, or no medical records who need to see a doctor for their undocumented problems. If you ave a life long condition, we can write you an authorization with free renewals for $500. TWSMMAA is very competitive, we will beat any price quote. As your primary physician, our doctors conduct a physical examination. If we determine you qualify, we will write you an authorization. In six months you can renew for a year for $150. We accept cash, credit, debit, and check. This is everything you will need to get started. What Medical Records Are We Looking For And How Do I Get Them?

If you want a one year authorization for $200, we need to get a copy of your medical record which states your qualifying conditions. This includes any relevant information related to the qualifying condition. In most cases we would like to see at least two or more years of medical history. We do not need your entire record. Just call us and we will help you get them. You will have to fill out a release form for them to forward your medical record to us. We recommend you go and pick them up from the records department, it is the fastest way.
